1. Introduction and Acceptance of Terms

These Terms of Service (referred to in this document as "these Terms") govern your use of services provided by Wunderlich Chiropractic and your use of our website at www.WunderlichChiro.com. Please read these Terms carefully before booking an appointment or using our services.

By booking an appointment — whether online, by phone, or in person — or by receiving treatment at our clinic, you confirm that you have read, understood, and agree to be bound by these Terms. If you are booking on behalf of a minor patient, your agreement also binds you in your capacity as parent or legal guardian, and you accept responsibility for ensuring the minor patient's compliance with these Terms where applicable.

If you do not agree to these Terms, please do not book an appointment or use our services. If you have any questions about these Terms before booking, please contact us using the details in Section 17.

We provide chiropractic care to patients of all ages, including children under 18. Our practice operates in accordance with the standards set by the Wisconsin Chiropractic Association and the Wisconsin chiropractic licensing requirements administered through the Wisconsin Department of Safety and Professional Services (DSPS).


3. Our Services

Wunderlich Chiropractic provides professional chiropractic services, which may include spinal assessment and adjustment, musculoskeletal evaluation, postural analysis, soft tissue therapy, therapeutic exercise guidance, and patient education relating to spinal and musculoskeletal health and wellness.

Individualized Treatment

Every patient is different, and all treatment plans are tailored to the individual based on their clinical presentation, health history, and goals. The treatment approach we recommend will be explained to you during your consultation and may evolve over time as your condition responds to care.

No Guarantee of Outcomes

While we are committed to providing the highest standard of care, chiropractic treatment — like all healthcare — cannot guarantee specific results. Individual outcomes depend on many factors, including the nature and severity of your condition, your overall health, and your engagement with any home care recommendations. We will always be honest with you about what we expect treatment can and cannot achieve.

Qualified Practitioners

All chiropractic services at Wunderlich Chiropractic are provided by a licensed chiropractor holding a valid Wisconsin chiropractic license issued by the Wisconsin Department of Safety and Professional Services. Our practitioner maintains the continuing education and professional development required to remain in good standing with their licensing board.

Scope of Practice

Chiropractic care is a recognized healthcare profession focused on the diagnosis, treatment, and prevention of disorders of the musculoskeletal system, particularly the spine, and their effects on the nervous system and overall health. Our services fall within the defined scope of chiropractic practice under Wisconsin law. We do not practice medicine, prescribe medications, or provide services outside the licensed scope of chiropractic practice.


4. Booking and Appointments

You can book an appointment with us using our online booking system, by calling us at 608-274-7660, or by emailing us at matt@wunderlichchiro.com. We do our best to accommodate your scheduling preferences and will confirm your appointment as promptly as possible.

Online Booking

Our online booking system allows you to schedule appointments at your convenience. When booking online, you will be asked to provide your name, contact details, and basic information about the reason for your visit. By completing an online booking, you confirm that the information you provide is accurate and that you agree to these Terms.

Appointment Confirmation

Once your appointment is booked, you will receive a confirmation via email or text message. Please review your confirmation carefully and contact us immediately if any details are incorrect. Appointment reminders may also be sent ahead of your scheduled visit as a courtesy.

Arriving for Your Appointment

Please aim to arrive a few minutes before your scheduled appointment time, particularly for your first visit, as there may be intake paperwork to complete. If you are a new patient, we recommend allowing extra time before your first appointment to complete any required health history forms. Arriving late may mean that your appointment time is shortened out of consideration for other patients, and the full appointment fee will still apply.

If We Need to Reschedule or Cancel

We will always try to give you as much notice as possible if we need to reschedule or cancel your appointment due to circumstances on our side, such as practitioner illness or an emergency. In such cases, we will contact you using the details you provided when booking and offer you the earliest available alternative appointment. We sincerely apologize for any inconvenience this may cause. If a rescheduled appointment does not suit you, any prepaid fees for that appointment will be refunded or credited to your account in full.

New Patient Consultations

Your first appointment will include a full health history intake and clinical assessment before any treatment is provided. This allows us to understand your condition fully and explain what care we recommend, including the likely risks, benefits, and alternatives, so you can make an informed decision about whether to proceed. No treatment will begin without your explicit consent.


5. Cancellation and No-Show Policy

We understand that unexpected situations arise and that you may sometimes need to change or cancel your appointment. We ask that you give us as much notice as possible so that we can offer your appointment slot to another patient who may be waiting for care.

Notice Period

We kindly ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment. You can cancel or reschedule by calling us at 608-274-7660, emailing matt@wunderlichchiro.com, or using our online booking system if cancellation is available through that platform.

Late Cancellations and No-Shows

At this time, Wunderlich Chiropractic does not charge a fee for late cancellations or missed appointments. We trust our patients to be considerate of our time and the time of other patients who may need care. However, we reserve the right to introduce a cancellation fee in the future with appropriate notice to patients. If we do so, these Terms will be updated and you will be notified before any fee applies.

Repeated No-Shows

While we do not currently charge a fee for missed appointments, patients who repeatedly fail to attend scheduled appointments without notice may be asked to prepay for future appointments or may, at our discretion, be discharged from our care. We will always communicate openly with you before taking any such step and will explain our reasoning.

Running Late

If you are running late for your appointment, please let us know as soon as possible by calling 608-274-7660. We will do our best to accommodate you, but depending on the length of the delay and whether other patients are scheduled after you, your appointment may need to be shortened or rescheduled. The full appointment fee will apply for shortened appointments caused by your late arrival.


6. Fees and Payment

Our fees are communicated to you at the time of booking and before your appointment begins. We believe in transparency around pricing, and you are always welcome to ask about fees before committing to any appointment or treatment plan.

Payment Methods

We accept the following forms of payment for our services:

• Cash

• Personal check

• Insurance billing (see below)

When Payment Is Due

Payment for your appointment is generally due at the time of your visit. If you are paying by check, please ensure that checks are made payable to Wunderlich Chiropractic. We reserve the right to decline a personal check in the event of a previously returned payment.

Insurance Billing

We accept insurance as a payment method and will submit claims to your health insurance provider on your behalf where you have authorized us to do so and provided valid insurance information. You are responsible for understanding your own insurance coverage, including any deductibles, copayments, or coinsurance amounts that apply. We strongly encourage you to verify your chiropractic benefits with your insurer before your first appointment.

Please be aware that insurance coverage is not guaranteed. If your insurer denies a claim or pays less than the full fee, you will be responsible for the outstanding balance. We will make reasonable efforts to work with you to resolve insurance disputes, but the ultimate financial responsibility for unpaid balances rests with you as the patient.

Unpaid Balances

If a balance remains unpaid after your appointment, we will contact you using the details on file to discuss payment. We may agree to a payment arrangement where appropriate. Accounts that remain significantly overdue despite our attempts to resolve them may be referred to a collections process. We will always attempt to resolve outstanding balances directly with you before taking any such step.

Fee Changes

We reserve the right to update our fees from time to time. We will provide reasonable notice of any fee changes, and updated fees will apply to appointments booked after the notice period, not to previously confirmed bookings.


7. Consent to Treatment

Your safety and autonomy are our highest priorities. Treatment at Wunderlich Chiropractic will only ever begin with your freely given, informed consent. This is not just a legal requirement — it is a fundamental part of the way we practice.

What Informed Consent Means

Informed consent means that before any treatment begins, we will explain to you in plain language what the proposed treatment involves, what we hope it will achieve, what the reasonably foreseeable risks and side effects are, what the alternatives are (including no treatment), and give you the opportunity to ask any questions you have. Only after you have understood this information and agreed to proceed will treatment begin.

How Consent Is Documented

For new patients, you will be asked to sign a written consent form before your first treatment. This form will be part of your clinical record and will be stored digitally in accordance with our Privacy Policy. We will revisit consent whenever we propose a new or significantly different treatment approach, or if your clinical situation changes in a way that affects the risk-benefit discussion.

Chiropractic Treatment Disclosure — Wisconsin

In accordance with Wisconsin state law and the standards of the chiropractic profession, we are required to inform you that chiropractic treatment, including spinal manipulation and adjustment, carries certain risks. These risks will be discussed with you in full during your consultation and are detailed in your consent documentation. Common risks associated with chiropractic care include temporary soreness or discomfort following an adjustment. Rare but more serious risks, including vertebrobasilar stroke in association with cervical manipulation, will also be disclosed and discussed. By proceeding with treatment, you acknowledge that these risks have been explained to you.

Your Right to Withdraw Consent

You have the right to withdraw your consent to treatment at any time, including mid-treatment, without any obligation to give a reason. If you wish to stop treatment, please tell us and we will stop immediately. Withdrawing consent does not affect your right to return for care in the future, and it will not affect the quality of care we provide to you if you choose to continue.

Consent for Insurance Billing

If you wish us to submit insurance claims on your behalf, you will also be asked to sign an Assignment of Benefits authorization, which allows us to bill your insurer directly and receive payment on your behalf. This is separate from your consent to treatment.


8. Patient Responsibilities

A successful clinical relationship works both ways. To help us provide you with the best possible care, we ask you to take the following responsibilities seriously. Failure to meet these responsibilities may affect the quality of care we are able to provide and, in some cases, your safety.

Providing Accurate Health Information

You must provide complete and accurate information about your health history, current symptoms, medications, supplements, previous treatments, and any other information that may be relevant to your care. This includes information that you may consider embarrassing or minor. The clinical decisions we make are based on the information you give us — incomplete or inaccurate information can affect your safety and the effectiveness of your treatment. We are committed to treating all information you share with us with the strictest confidentiality.

Notifying Us of Changes

You are responsible for informing us of any changes in your health, medications, or circumstances between appointments. If you have a new diagnosis, have started or stopped any medications, have been involved in an accident, have undergone surgery, or have noticed any new or worsening symptoms, please tell us before your next appointment begins. This allows us to review your treatment plan and ensure that it remains safe and appropriate for you.

Following Clinical Advice

We may recommend home care activities such as stretching, exercise, ice or heat application, postural changes, or activity modifications as part of your treatment plan. While we respect your autonomy and will never force you to follow advice you disagree with, your outcomes are more likely to be positive if you engage with the full treatment plan, including any home care recommendations. Please discuss any concerns or difficulties with home care recommendations openly with us.

Raising Concerns Promptly

If you experience any unexpected symptoms, worsening pain, or side effects following treatment, please contact us as soon as possible. Do not wait until your next scheduled appointment if you are concerned about your condition. If you have a medical emergency or believe you are experiencing a serious adverse reaction, please call 911 or go to your nearest emergency room immediately.

Respectful Conduct

We are committed to providing a safe, respectful, and inclusive environment for all patients and staff. We ask that all patients treat our staff and fellow patients with courtesy and respect. We reserve the right to decline to treat or to discontinue treatment for any patient whose behavior is abusive, threatening, or disrespectful toward staff or other patients.


9. Minors and Patients Under 18

We welcome patients of all ages at Wunderlich Chiropractic, including children and adolescents. We take the safety and wellbeing of our younger patients very seriously and apply additional care and protocols to their treatment.

Parental or Guardian Consent

Patients under the age of 18 must be accompanied to their appointments by a parent or legal guardian. The parent or legal guardian is responsible for providing informed consent to treatment on the minor patient's behalf before any treatment begins. We will explain the proposed treatment, its risks and benefits, and any alternatives to the accompanying adult in the same way we would explain it to an adult patient, and we will also involve the minor patient in the discussion in an age-appropriate way.

Documentation of Consent

Consent for a minor patient will be recorded in writing and signed by the parent or legal guardian. This signed consent will be retained as part of the minor patient's clinical record in accordance with our record retention policy.

Ongoing Parental Authorization

Where a minor patient attends appointments regularly, a parent or legal guardian may provide general ongoing consent to a course of treatment. However, we will seek renewed consent whenever the treatment plan changes significantly or when a new type of treatment is proposed. We may ask for a parent or guardian to be present for certain appointments, particularly for younger children.

Adolescent Patients

Adolescent patients are encouraged to participate actively in discussions about their own care. In certain circumstances, Wisconsin law permits minors to consent to certain types of healthcare independently. Where this applies, we will respect the adolescent patient's privacy rights in accordance with applicable law and our Privacy Policy.

Health and Safety Information

A parent or guardian authorizing treatment for a minor patient takes on the same responsibilities as described in Section 8, including providing complete and accurate health information about the minor and notifying us of any changes in the minor's health or circumstances.

10. Limitation of Liability and Clinical Disclaimer

We provide our services with care, professionalism, and in accordance with current evidence-informed chiropractic practice standards. However, there are important limitations on our liability that you should understand before receiving care.

Inherent Risks of Chiropractic Treatment

All healthcare interventions, including chiropractic treatment, carry inherent risks. Common side effects following chiropractic adjustment may include temporary soreness, stiffness, or fatigue, which typically resolve within 24 to 48 hours. More serious adverse events are rare but possible, and these will be fully explained to you during the informed consent process before any treatment begins. By consenting to treatment, you acknowledge that you have been informed of and accept these risks.

We Are Not a Substitute for Emergency or Primary Medical Care

Chiropractic care is a complement to, not a replacement for, primary medical care or emergency medical services. If you are experiencing a medical emergency, chest pain, difficulty breathing, sudden severe headache, signs of stroke, or any other life-threatening symptoms, please call 911 or proceed immediately to your nearest emergency room. Our services are not appropriate as a substitute for emergency care, and we are not equipped to manage medical emergencies.

Limitation of Liability

To the maximum extent permitted by Wisconsin law, Wunderlich Chiropractic's total liability to you for any claim arising from or related to the services we provide shall not exceed the fees actually paid by you for the specific treatment session or sessions to which the claim relates. This limitation does not apply in cases of gross negligence, willful misconduct, or where applicable law does not permit such limitation.

We do not exclude or limit our liability for personal injury or death caused by our negligence, or for any other type of liability that cannot be excluded or limited under Wisconsin law or applicable federal law.

No Liability for Third-Party Actions

We are not responsible for the actions, omissions, advice, or recommendations of any third-party healthcare providers to whom we may refer you. Any referrals we make are provided in good faith based on your clinical needs, but the decision to consult any third party is yours, and we have no control over the care they provide.

Patient-Provided Information

We rely on the accuracy of the information you provide to deliver safe and appropriate care. We are not liable for adverse outcomes that result from treatment decisions made on the basis of inaccurate, incomplete, or withheld information provided by you.


11. Professional Standards and Regulation

Wunderlich Chiropractic operates in full compliance with the professional, ethical, and legal standards governing chiropractic practice in the State of Wisconsin. The following regulatory framework applies to our practice.

Wisconsin Chiropractic Licensing

Chiropractic practice in Wisconsin is regulated by the Wisconsin Department of Safety and Professional Services (DSPS) under Wisconsin Statutes Chapter 446. All chiropractors practicing in Wisconsin must hold a valid license issued by the DSPS, comply with continuing education requirements to maintain that license, and practice within the defined scope of chiropractic practice as established under Wisconsin law.

Professional Code of Conduct

Our chiropractor adheres to the professional and ethical standards established by the Wisconsin chiropractic licensing laws and the standards of the chiropractic profession. This includes standards relating to patient consent, record-keeping, scope of practice, advertising, and professional conduct.

Wisconsin Chiropractic Association

Wunderlich Chiropractic may hold membership with the Wisconsin Chiropractic Association (WCA), which supports professional development and high standards of practice across the chiropractic profession in Wisconsin.

Verification of Credentials

You are entitled to ask us about our practitioner's qualifications, license status, and professional standing at any time. Chiropractor license status in Wisconsin may also be independently verified through the Wisconsin DSPS online license lookup tool at dsps.wi.gov.


12. Complaints Procedure

We are committed to providing you with an excellent experience at every visit. However, we understand that concerns and complaints can arise, and we take all feedback seriously. If you are unhappy with any aspect of your care or experience, we encourage you to tell us — it helps us improve and gives us the opportunity to put things right.

Raising a Concern With Us Directly

In the first instance, please bring your concern to our attention as soon as possible. You can do this by speaking with us directly during or after your appointment, calling us at 608-274-7660, or sending an email to matt@wunderlichchiro.com. We will acknowledge your concern promptly and aim to provide a full response within 14 business days. If the matter is complex and requires more time, we will let you know and keep you updated on progress.

Formal Written Complaints

If you would prefer to make a formal written complaint, please send it to us by email or by post to Wunderlich Chiropractic, 2940 Chapel Valley Rd., Fitchburg, WI 53711. Please describe the concern clearly, including relevant dates and details, so we can investigate thoroughly. We will confirm receipt of your written complaint within five business days and aim to provide a substantive written response within 30 days.

Escalating Your Complaint — Wisconsin DSPS

If you are not satisfied with the outcome of our internal complaints process, or if you have a concern about the professional conduct or competency of our chiropractor that you wish to report to a regulatory authority, you have the right to file a complaint with the Wisconsin Department of Safety and Professional Services (DSPS). The DSPS has the authority to investigate complaints against licensed chiropractors in Wisconsin.

These Terms of Service and any dispute or claim arising from or in connection with them — including disputes relating to their formation, validity, interpretation, breach, termination, or enforceability — are governed by and construed in accordance with the laws of the State of Wisconsin, United States of America, without regard to its conflict of laws principles.

Any legal action or proceeding arising out of or relating to these Terms or your use of our services shall be brought exclusively in the state or federal courts located in Dane County, Wisconsin. By using our services and agreeing to these Terms, you consent to the personal jurisdiction of those courts. Nothing in this clause prevents either party from seeking urgent injunctive or other equitable relief in any court of competent jurisdiction.

If you are a consumer and applicable consumer protection law in your jurisdiction provides you with rights that cannot be waived by contract, those rights are not affected by this clause.
